RESUMO

Los neuromas del conducto biliar se desarrollan a partir de las fibras nerviosas simpáticas y parasimpáticas que envuelven la pared de la vía biliar. Mujer de 44 años de edad con antecedente de colecistectomía convencional seis meses previos al ingreso acude a emergencia por ictericia obstructiva de 15 días de evolución. En los estudios de imagen impresiona la presencia de una masa a nivel de las vías biliares considerándose el diagnostico de una neoplasia maligna. Por los antecedentes, ausencia de marcadores tumorales se decide realizar una biopsia percutánea sin resultados concluyente, realizándose posteriormente una intervención quirúrgica con estudio anatomo-patológico compatible con neuroma de amputación de vía biliar. El neuroma de amputación a nivel de la vía biliar es un tumor infrecuente. Puede manifestarse clínicamente como una ictericia obstructiva y suele simular a un tumor maligno de las vías biliares. El manejo quirúrgico es el tratamiento definitivo.


Neuromas of the bile duct develop from the sympathetic and parasympathetic nerve fibers that surround the wall of the bile duct. A 44-year-old woman with a history of conventional cholecystectomy six months prior to hospital admission attended emergency due to obstructive jaundice that lasted 15 days. In the imaging studies, the presence of a mass at the level of the bile ducts is considered, considering the diagnosis of a malignant neoplasm. Due to the antecedents, the absence of tumor markers, it was decided to perform a percutaneous biopsy without conclusive results, performing later a surgical intervention with anatomopathological study compatible with neuroma of biliary tract amputation. The amputation neuroma at the level of the bile duct is an infrequent tumor. It can manifest clinically as obstructive jaundice and usually simulates a malignant tumor of the bile ducts. Surgical management is the definitive treatment.

Objetivos: la encefalopatía de Wernicke es un desorden severo causado por deficiencia de vitamina B1. Esta enfermedad rara vez diagnosticada, es más prevalente de lo que se reporta, por lo que nuestro objetivo es presentar un caso inusual y hacer una revisión de la literatura.Materiales y métodos: se presenta el caso de una mujer gestante de 29 años de edad, con tiempo de gestación de 12 semanas, que ingresa al servicio de Emergencia del Hospital Macional Hipólito Unanue, hospital de referencia a nivel nacional, ubicado en Lima, Perú, que atiende pacientes de bajo nivel socioeconómico. Como sintomatología presentaba náuseas, vómitos incoercibles, amaurosis nocturna con manifestaciones psicóticas, ataxia y baja ponderal del 12% de peso. Se diagnosticó por bajos niveles séricos de vitamina B1. Para la búsqueda sistemática se utilizaron las palabras clave: Síndrome de Wernicke Korsakoff e Hiperemesis gravídica en bases de datos Pubmed, Scopus, Embase, DOAJ, SciELO, Latindex, Redalyc y Lilacs. Resultados: se encontraron 135 artículos, de los cuales 45 estaban relacionados directamente con el tema, de estos se seleccionaron 6 reportes de caso y 11 revisiones del tema. Conclusiones: la enfermedad de Wernicke Korsakoff asociada a hiperémesis gravídica es de difícil diagnóstico, y necesita manejo rápido y oportuno para conservar la salud de la madre y el feto


Objectives: Wernicke’s encephalopathy is a severe disorder caused by vitamin B1 deficiency. The prevalence of this rarely diagnosed disease is higher than reported. Consequently, our objective is to present an unusual case and conduct a review of the literature. Materials and methods: We present a case of a pregnant woman, 29 years old, who presented at 12 weeks of gestation to the emergency service of the Hospital Nacional Hipólito Unanue, a tertiary referral center in Lima, Peru, that serves patients in the low-income bracket. Symptoms included nausea, uncontrolled vomiting, night blindness with psychotic manifestations and ataxia, and low weight in the 12% range. Diagnosis was based on serum levels of B1 vitamin. A systematic search was conducted using the following key words: Wernicke Korsakoff syndrome and hyperemesis gravidarum in Pubmed, Scopus, Embase, DOAJ, SciELO, Latindex, Redalyc and Lilacs. Results: The search resulted in 135 articles, 45 of which were directly related to the topic. Of those, 6 case reports and 11 review articles were selected. Conclusions: Wernicke-Korsakoff disease associated with hyperemesis gravidarum is relatively difficult to diagnose, and requires prompt and timely management in order to preserve the health of the mother and the fetus

La endometriosis es una condición en que el tejido endometrial es localizado fuera de la cavidad uterina. Esta puede ser pélvica o extra pélvica, y de esta última, la afectación del tracto gastrointestinal representa de 3-15% y de localización más frecuente en el rectosigmoides, siendo el compromiso apendicular raro con 3-5% de todas las endometriosis, motivo por el cual se presenta este caso, diagnosticado incidentalmente en una paciente admitida en emergencia por cuadro de apendicitis aguda.


Endometriosis is a condition in which uterine mucosal tissue is located outside the uterus. Endometriosis may be pelvic or extrapelvic, and in this latter case, gastrointestinal tract involvement represents 3-15% of cases, being most frequently located in the rectal-sigmoid region. Appendix involvement in endometriosis is rare, accounting 3-5% of all endometriosis cases. For this reason we present this case of appendicular endometriosis, which was incidentally diagnosed in a patient admitted in the emergency service with a condition resembling acute appendicitis.
